Chronically Supported CIC is a community interest company led by people living with chronic illness and supporting people with chronic illness. We understand how difficult it can be to navigate the changes to your life that being diagnosed brings and offer both group support and 121 support.
Our 121 support can help you with administration that you can no longer manage, support on phone calls, company from someone that understands what living with a chronic condition is like, occasional appointment support. Having this support can help you feel less overwhelmed by life admin and less socially isolated. You will benefit from the sign-posting, support and other services we provide in our group sessions.
