To create opportunities for the people you support to develop and maintain friendships and personal relationships, encouraging maximum participation and inclusion within their communities.
To achieve a range of appropriate communication techniques in accordance with the individual needs of the person.
To provide support to people in their contacts with healthcare specialists, social services, housing departments and other external agencies, e.g. attending relevant appointments, helping to explain information, and completing relevant paperwork in partnership with individuals.
To ensure that people receive all necessary advice, care and regular health checks to maintain their physical health and well-being.
To promote nutrition, relaxation, exercise and a healthy lifestyle.
