Lydia’s Help aims to help older adults in Croydon transitioning from hospital to home by preparing their living spaces, offering personalised practical and emotional support, and giving carers the time and reassurance they need to rest and recharge. We bridge the gap between medical care, reablement care and full recovery.

Pre- Discharge/Home Readiness

Home Recovery and Carer Support
Many older people struggle after hospital discharge, and carers often feel exhausted keeping up with recovery needs. Our home recovery and carer support service bridges that gap by providing gentle practical help and companionship and giving carers time to rest. The result is safer recovery, reduced stress , and greater independence.
