At Pblossoms Healthcare Services (PHCS), we are dedicated to providing compassionate, person-centred care tailored to empower individuals aged 18 and over to live as independently as possible. My services encompass a wide range of support, including personal, social, mental, and psychological care for those facing challenges such as self-neglect, disabilities, and mental health conditions. As a qualified Infection Prevention and Control Nurse and Registered Mental Health Nurse with qualifications in Health and Social Care Management, Mentorship, and Smoking Cessation, I have been providing dedicated healthcare services since 1995. I am dedicated to delivering compassionate, person-centred care that empowers individuals to live as independently as possible, with collaboration among families, friends, and healthcare providers as needed. I maintain professional boundaries while supporting each person with their unique health and social care needs.

My Services:

• PHC Services: Tailored to individuals including those experiencing self-neglect, disability, smoking or substance misuse, and various physical and mental health conditions.
• Assertive Outreach: Engaging people with longstanding mental and physical health needs who may struggle with consistent care.
• Enablement Services: Short-term support to help clients learn or regain skills for independence, connecting them to additional services where necessary.
• Respite for Family Caregivers: Providing relief and support to help caregivers manage their responsibilities.
• Community Access: Assistance in engaging with local community resources.
• Household Support: Light cleaning, shopping, meal preparation, telephone support.

With a commitment to understanding, and offering professional and compassionate care that addresses each patient’s and client’s unique needs, I aim to empower individuals to live their best lives helping them thrive in the comfort of their own homes.
