PEAR CARE

Hello, my name is Pauline, I live in a small  border town near Presteigne in Powys.

I have worked within the NHS for many years and have a wealth of experience working with community nurses and GPs as Support /Senior HCA /Assistant Practitioner and Ward based,  more recently with Community Occupational and Physiotherapists, helping with rehabilitation of patients at home.

My experience also  covers working with the elderly and patients with dementia, I am a Dementia Champion, I have also worked with patients with disabilities, at present I work on a part time basis as a Therapy Support Worker with a local NHS community team, delivering and supporting patients reablement programmes at home after leaving hospital, a period of illness or after surgery, empowering patients and families to participate in the management of their illness or disability, working flexibly with the indirect supervision of a qualified therapist.

I have worked with patients at the end of life, at home and in a hospice. I  have my level 3 in End of Life Care, I also have my level 4 in Integrated Care and level 2 Dementia care, alongside the Social Welfare diploma.

I enjoy working alongside people in their own homes helping them to achieve a good fulfilling quality of life in their own environment within the community, so I have decided to offer my services to help enable and care for people, encompassing all the experience I have acquired over the years, this is a summary of the services I can offer, each individuals’ care will be personalised to suit their own circumstances. I have a current enhanced DBS and liability insurance.
