Ask to see a provider’s portfolio

This symbol means that the enterprise leader has a portfolio (folder) containing all the elements that show how they meet our Doing It Right Standards. It will provide you with information on the way that they work, checks they have in place, policies, procedures, and more. You can ask to see it to help you make any care choices.

Hi, my name is Fleur aka The Form Buddy

I have worked in Cheshire West and Wirral for over 7 years supporting people to live their best life. I have experience of working within carer organisations, and also the NHS covering long Covid, respiratory, stroke support and heart failure / cardiac rehab.

I can help you with completing complex forms, however, I am also happy to help you with tasks such as a Compare the Market search for your house insurance! I love sorting things out and can help organise your paperwork so it is easy for you to access.

I am also happy to help out with lunch or a bit of companionship, I love a good chat, and looking through old photos!

In my spare time I enjoy walking, shopping, socialising and an occasional gin and tonic!

The kind of services I offer are:

LPA (Lasting Power of Attorney) forms

PIP (Personal Independence Payment) forms  – including Mandatory reconsideration and Tribunals

AA (Attendance Allowance) forms

DLA (Disability Living Allowance) forms

CA (Carers Allowance) forms

Blue badge forms

Any other form completion within reason

Organising paperwork

Meal preparation

Companionship

Shopping

Trips out

Please feel free to give me call and we can have a chat to see if I can help.

I welcome direct payment customers and I am happy with the rate

I have personal experience of supporting Neurodiverse individuals. I have also supported people with long-term health conditions, long Covid, chronic fatigue and poor mental health.
