VKREMOYE Wellbeing & Compassionate Care Services is a nurse-led community wellbeing enterprise founded to support adults to live healthier, safer, and more independent lives within their own homes and communities.
The service is led by a Registered Nurse with extensive NHS experience in pre-assessment, outpatient care, patient safety, and clinical governance. This clinical background ensures that all support delivered is safe, compassionate, person-centred, and evidence-based.
Our enterprise focuses on providing flexible, preventative, and non-medical community support that addresses the wider determinants of health, including loneliness, social isolation, reduced independence, and post-discharge vulnerability.
We offer a range of services including:
Wellbeing checks and welfare visits
Companionship and befriending support
Hospital discharge and recovery support
Emotional wellbeing and practical support
Dementia-friendly companionship
Carer support and respite assistance
Community-based health promotion and coaching
We also have a specialist focus on supporting individuals following hospital discharge, helping them to remain safe, understand care plans, and access appropriate community services to reduce the risk of readmission.
Our approach is strengths-based, person-centred, and community-focused. We work collaboratively with families, carers, GP practices, social prescribers, voluntary organisations, and statutory services to ensure individuals receive coordinated and compassionate support.
