We are excited to be working with Age Connects Morgannwg (a charity for older people living in Rhondda Cynon Taf, Bridgend, and Merthyr Tydfil) and their team of Trusted Assessors who are based at our district general hospitals. The Trusted Assessors play a key role in getting people home from hospital. Together, we’ll be focusing on how micro-enterprises can support people who need ongoing support to keep them safe at home this winter.
