The Personalised Care Training Academy is the training arm of Pathways Community Interest Company, whose vision is to provide outstanding Personalised Care training and development solutions.
The Academy delivers both off-the-shelf Personalised Care Institute accredited programmes as well as bespoke outcome-focused learning programmes, designed to meet individual organisational needs.
C4PC Programme Manager, Emma Geldart, told us “As well as training, the Personalised Care Training Academy delivers measurable skills and knowledge that health and social care staff need.
“This supports the implementation of the NHS Long Term Plan and Comprehensive Model for Personalised Care, and helps to ensure that Personalised Care becomes ‘business as usual’.”
The Personalised Care Training Academy is focused on reducing health inequalities and supporting patients to understand how they can better manage their health and improve their quality of life. Their social prescribing services work directly with Primary Care to embed Personalised Care into their everyday practices.
Josh Matthews, Personalised Care Training Academy Operations Manager, explained their reasons for joining the Coalition:
