Supporting you to Thrive joins C4PC as a new Partner

Supporting You to Thrive is an organisation specialising in providing Community Connection and Support Brokerage to people who are 16 and over with a Learning Disability, Autism, Physical Disability, Brain Injury or living with Dementia.

Glen Richards, Supporting you to Thrive Director, explains that, “Community Connection is our preferred term for what is widely known as ‘social prescribing’, as we feel these words have more relevance to what we actually do. We understand that for some people it is not possible to be simply offered information about activities and then be expected to go it alone.

“We recognise that for some people there are barriers which prevent them from making these steps forward, and we work with people to identify what matters to them and encourage and support them to connect with their community.”

With over 20 years of experience working within health and social care, Glen is a strong advocate for people who are among the most affected by health inequalities.

C4PC Programme Manager, Emma Geldart, tells us, “Glen’s passionate belief that everyone has the right to be heard and to be listened to are at the core of what makes Personalised Care so important.

“The experience and connections Supporting you to Thrive bring to the Coalition will be a great asset, and help us to strengthen our focus on enabling people to lead a good life where they thrive, not just survive.”
